This form must be filled out and signed. Upon reception of the form, the RMA agent will fill out an entry voucher for our equipment. Once the RMA is completed in our office, a fax confirmation will be sent to you, within 60 minutes. Upon reception of this confirmation, all you need to do is put the pre-addressed tag, with the return number in full sight, on your equipment's box and ship it.

Work order
A work order, describing the type of repair or estimate,
goes with the voucher for your equipment.
An estimate is usually done within 24 to 72 hours. The fee
for the estimate depends on the type of equipment. Please
contact our customer service department.
30 days to 48 months, depending on the type of product or
the repair to be done.
We always use the customer's shipping company, unless it is
not available in our area, in which case we use the following:
Bills can paid in 10 days (upon approbation from our credit department),
by cash, check or wire transfer.
